In hemodynamically stable, alcohol-intoxicated blunt-trauma patients who are alert (GCS = 15) and have a normal abdominal examination, abdominal CT detected injuries in only 2 % of cases and changed management in just 0.6 %, indicating that routine CT is not always necessary [Mavridis & Firilas, 2000, PMID 10798511]. Similar findings were reported earlier, with 0 of 75 asymptomatic, intoxicated patients showing intra-abdominal injury despite CT/DPL, supporting selective rather than mandatory imaging [Perez et al., 1991, PMID 2024788].
