Levetiracetam is recommended as first-line monotherapy for new-onset focal seizures in the elderly because it has good efficacy, few drug–drug interactions, and better tolerability/retention than carbamazepine or valproate, whereas oxcarbazepine often causes hyponatremia in this age group and pregabalin is not an established first-line agent [Werhahn, 2015, PMID 25684224; Pohlmann-Eden, 2016, PMID 27552848].
