Clay-coloured (acholic) stools in a jaundiced neonate point to obstructive cholestasis from extra-hepatic biliary atresia, and liver biopsy in biliary atresia commonly shows multinucleated giant-cell transformation of hepatocytes, a feature also seen in neonatal hepatitis but consistent with the atresia diagnosis here [Bill 1977, PMID 592078; Mowat 1976, PMID 1087549].
