The combination of social unresponsiveness, stereotyped movements (rocking, hair-twisting), and self-injurious behavior (fist-hitting) are hallmark features of autism spectrum disorder, not typical of simple dental anxiety, isolated intellectual disability, or an ictal event. Children with autism commonly display repetitive motor stereotypes and self-injury alongside apparent indifference to social cues or name-calling [Tordjman et al., 1999, PMID 10370885; Berkson, 2002, PMID 12323071].
