A patient who already has confirmed extra-pulmonary TB is not a “TB suspect”; they are an established case requiring treatment. The RNTCP defines a presumptive (suspect) TB case as: 1) any person with cough ≥2 weeks, 2) people living with HIV with cough of any duration, and 3) household contacts of a smear-positive case with cough of any duration, among others [Thomas, 2008, PMID 18516823; Kumar, 2016, PMID 27244055]. Hence options B, C and D all fit the suspect definition, while option A does not.
