Thyroglossal fistula represents a persistent tract of the thyroglossal duct; histologically its wall is most often lined by stratified squamous (sometimes respiratory columnar) epithelium, confirming option A [Thompson et al., 2016, PMID 27161104; Satou et al., 1989, PMID 2489462]. It is a congenital anomaly, not primarily inflammatory, does not itself create a neck bulge (that is typical of a cyst), and has no intrinsic premalignant nature.
