Excessive occlusal forces create localized zones of pressure necrosis in the periodontal ligament; these “necrotizing pressure areas” are followed by osteoclastic bone resorption on the socket wall and compensatory endosteal (inner-surface) bone formation—classic microscopic findings of trauma from occlusion, not of inflammatory or ulcerative periodontal diseases [Torisu, 1989, PMID 2489655; Wang, 2018, PMID 30299110].
