Phenylephrine is a potent sympathomimetic mydriatic; pharmacologic dilation of a predisposed narrow angle can precipitate acute angle-closure, and phenylephrine is specifically listed as contraindicated in such patients [Meyer & Fraunfelder, 1980, PMID 7243210]. Timolol and acetazolamide are used to lower intra-ocular pressure, while homatropine is seldom used but is not as uniformly contraindicated as phenylephrine.
