High-velocity gunshot fractures strip periosteum, leave devitalized bone, and are heavily contaminated, so current protocols recommend aggressive initial debridement with temporary stabilization, followed later by definitive load-bearing plating and bone graft once the soft-tissue envelope is healthy (staged reconstruction) [Toogood & Miclau, 2017, PMID 28938387; Watson et al., 1995, PMID 7634662].
