The classic peripheral-smear triad of megaloblastic anemia consists of macro-ovalocytes and anisopoikilocytosis, together with hypersegmented neutrophils; reticulocyte counts are normal or low because DNA-synthesis failure leads to ineffective erythropoiesis rather than brisk marrow response. Thus reticulocytosis is not part of the triad, whereas Howell-Jolly bodies, hypersegmented neutrophils and macro-ovalocytes may all be seen. [Davenport, 1996, PMID 8546042]
