High-intensity statin therapy is recommended for all adults ≤75 years with established atherosclerotic cardiovascular disease (e.g., recent MI); appropriate doses are atorvastatin 40–80 mg or rosuvastatin 20–40 mg. Adding a fibrate is reserved for severe hypertriglyceridemia (≥500 mg/dL) or persistent elevations despite statin therapy, and fenofibrate alone would not address the elevated LDL. Therefore, atorvastatin 80 mg is the preferred choice for this patient [Grundy, 2019, PMID 30586785].
