Petechial spots that appear distal to a pressure cuff in a young adult with sore throat, headache, vomiting and hypotension point to meningococcal septicemia; Neisseria meningitidis classically produces a rapidly progressive illness with shock and a petechial/purpuric rash in this age group [Gradaus et al., 1999, PMID 10603735].
