Troponin level is integral to MI classification (NSTEMI vs. unstable angina) and informs the need for intensified antithrombotic therapy and early invasive management; guideline reviews show patients with elevated troponin benefit most from aggressive treatment [Scirica & Morrow 2003, PMID 15199443; Liebetrau 2017, PMID 28233037]. Regional wall-motion abnormalities help confirm diagnosis and assess prognosis but do not, by themselves, direct acute MI therapy, so they are not considered a primary treatment-modifying factor.
