Aortic stenosis produces severe pressure overload of the left ventricle and is a classic cause of isolated left-sided (pulmonary-congestive) heart failure [Mengi, 2024, PMID 39412797].
Patent ductus arteriosus creates a large systemic-to-pulmonary shunt, leading to volume overload of the left atrium and ventricle and can likewise present with pure left-sided failure [Adler, 2013, PMID 24094805].
By contrast, atrial septal defect and pulmonary valve obstruction predominantly overload the right heart, so they do not cause isolated left-sided failure.
