Isolated rotation of a maxillary central incisor in an otherwise well-aligned occlusion is classically associated with the presence of a palatally impacted mesiodens or other supernumerary tooth; therefore, the first step is to radiographically evaluate for such a tooth before planning any orthodontic mechanics [Giancotti, 2002, PMID 11990044; Ayers, 2014, PMID 24994110].
